FOXO4-DRI is a peptide with D-amino acids and a retro-inverted sequence that acts as a senolytic โ a molecule that selectively eliminates senescent cells. Senescent cells lose functionality but evade apoptosis and continue secreting pro-inflammatory mediators โ the so-called senescence-associated secretory phenotype (SASP): IL-6, IL-8, TNF-ฮฑ, MMP โ gradually poisoning surrounding tissues. In van Deursen's original work in progeroid mice, FOXO4-DRI restored physical performance (running, grip strength), improved renal glomerular condition and coat quality, and increased precursor cell density in intestinal crypts โ with complete absence of toxicity to normal tissues. In a study on human chondrocytes, the peptide selectively removed senescent cells from culture without killing normal cells, making it promising for ACI protocols (autologous chondrocyte implantation).
Later work demonstrated sensitivity of keloid skin fibroblasts to FOXO4-DRI: senescent fibroblasts supporting pathological scar growth underwent apoptosis, opening prospects for use in hypertrophic scars. Large human RCT data are absent, keeping the molecule in research-agent status. Nevertheless, the mechanistic precision and preclinical evidence for FOXO4-DRI distinguish it among most senolytic candidates.
