HEP-1 (Human Ezrin Peptide 1) is a synthetic 14-amino-acid peptide reproducing amino acids 324–337 of the ezrin protein — a cytoskeletal protein linking the cell membrane to the actin cytoskeleton and participating in T-cell immune synapses. This region, called the Hep receptor, forms a double antiparallel alpha helix with alternating charged residues — a structure that plays a role in activation of the adaptive immune response. By binding to this ezrin region, HEP-1 modulates conformational changes in the protein and initiates downstream signaling events that enhance immune activity.

As an immunomodulator, HEP-1 enhances adaptive immunity through B- and T-cell arms: it increases antibody titers after hepatitis B vaccination, strengthens lymphocyte proliferative response, and normalizes subpopulation ratios in immunodeficient states. However, the molecule is not a full immunostimulant — it is more accurately described as an immunoregulator: it "brings up" a reduced immune response to normal without causing hyperactivation in a normally functioning system. Hepatoprotective potential is also described through protection of hepatocytes from toxic load and reduction of inflammatory cytokines in liver tissue.

In clinical development, HEP-1 (trade name Gepon) was used in viral infections, immunodeficient states, and after toxic burden, demonstrating a favorable tolerability profile. A clinical protocol for COVID-19 (NCT04627233) was initiated, evaluating immunomodulatory effect in severe infectious conditions. The evidence base remains predominantly observational and requires additional RCTs; however, the molecular mechanism and application experience are described in considerable detail.
