Thymosin Alpha-1 (Tα1) is a 28-amino-acid acetylated peptide originally isolated by Goldstein from the thymus as a factor restoring immune function in thymectomized animals. It is endogenously produced in the thymus and participates in T-lymphocyte maturation: from precursors, functionally mature CD4+ and CD8+ T cells form with full cytotoxic and helper activity. The molecular entry point is Toll-like receptors TLR2, TLR4, TLR7, and TLR9 on dendritic cells and macrophages: through them, Tα1 activates NF-ĪŗB, IRF3, and MAPK cascades, triggering synthesis of IFN-γ, IL-2, and IL-12 — cytokines coordinating antiviral and antitumor immune responses.

Regulatory status is confirmed: Tα1 is approved in more than 35 countries (trade name Zadaxin) for chronic hepatitis B and C; in China it was widely used in severe infections including COVID-19, demonstrating mortality reduction in retrospective analyses. Immunostimulatory use in oncology is also documented: enhancing vaccination response, restoring immune surveillance after chemotherapy-induced immunosuppression, and normalizing the "aged" immune phenotype — elevated CD8+/CD4+ ratio and reduced NK cell activity in elderly individuals. Separately, Tα1 increases intracellular glutathione in immune cells, adding an antioxidant dimension to the immune effect.

Mechanism pleiotropy is both a strength and complexity of the molecule: with reduced immunity it activates; with hyperactivated immunity (autoimmune conditions) it normalizes without causing excessive stimulation. This makes Tα1 more of an immune regulator than a simple stimulant. Tolerability is excellent — serious adverse events were virtually absent in clinical trials.
